CANCELLATION PROCEDURES IN RESPECT OF THE EVIDENCE(S) OF INSURANCE:

1 Cancellation By Participating Member — All cancellation requests must be submitted in writing to Azimuth Risk Solutions To be eligible for a full refund, the request must be received before the Participating Members requested Effective Date. Cancellation requests received after the requested Effective Date will be subject to the following:

1.1 A written request submitted to the Scheme Administrator by email, mail or fax; and

1.2 Request must be received prior to the requested cancellation date; and

1.3 Only the unused portion of the Premium cost will be refunded; and

1.4 If there are Pending claims that are Eligible, a refund will not be issued; and

1.5 A $25.00 cancellation fee
