• Ages – 18 year to 64 years.
• ID card - Apply online to print ID-card online.
• Renew - Renew online, by phone, by email, by fax.
• Covered Illness : Diagnoses must be made within the United States while the insured person’s insurance is in force and must be covered under the Policy
• Coverage Ends:  Date Premium is not paid when due or within 31 grace period, written request is received from the insured person, Insured person reaches max benefit, death of insured person, Insured person is no longer permanent or legal resident,  insured or spouse attains age 75.
